THANK YOU! I’ve been doing Keto for 3 weeks and I don’t think this is the diet nor lifestyle for me. I’ve been having on and off Keto Flu that included painful headaches and constant bowel issues. I thought it would be over by the end of week 1, but I continued so I thought again at the end of week 2, but continued. It’s the end of week 3 and I’m not seeing any changes. I’m feeling just as tired if not more than when I wasn’t on Keto. My mood has been more snappy and aggressive than when I wasn’t on Keto. I just wasn’t happy. I already was super sensitive to salt & salty foods and now I’m even MORE sick of salt & salty foods. I missed my fruits and veggies like carrots which my pup & I often snacked on. I got super gassy and burped more these past 3 weeks than my entire life. My period was a mess and came 4 days earlier than normal and I haven’t had an odd period in over a year after getting diagnosed with mild PCOS. Attempting Keto really threw my body in for a loop and it’s been down hill ever since. I haven’t dropped in weight, the inches are still there, the bloating is still there. But now I have this fear of eating. I used to miss meals before and did IF for a while but now I miss even MORE meals and rarely eat which is just a recipe for weight gain when you’re consuming high fat. I’ve stocked my fridge with items I rarely ever bought: Sour Cream, Bacon, Cream Cheese, Heavy Whipping Cream. All because I needed to hit my fat macros.
I do need to lose weight, but I have ALWAYS been the biggest Keto nay-sayer. It defied everything I believe in. What turned the tables for me after a lot of research is that it's supposed to help with infertility and at this point, I'm desperate and will try anything. So, I gave it 30 days and said I would re-evaluate whether I want to continue. And let me tell you, I want to continue. This is what it's done for me so far:

I got gout from drinking Vitamin Water-32 grams of sugar per bottle which equals 8 tsp. of sugar per bottle. Next time you buy pop or fruit juice or some processed food check for carbs. Our western diet is turning us into an obese society full of diseases. I find this very sad especially for the next generation. I truly believe a low carb diet could turn this around. I love Keto and all the great recipes and I will stay on it for the rest of my life. I am now off all medication and feeling great.
I read your blog on the keto diet and at the end, I cried, because your story is amazing. I dieted all my life, from Atkins, Paleo, keto And Many other diets. I always thought losing weight, would make people love me more or be impressed by my weight lose. Finally at the age of 64, I realize I need to just love myself and not worry what others think of me. You indicate that exogenous ketones do not shut down the ability, of your body, to oxidize fat. Is that to say it does not have an effect on your body at all? My specific question is… does my body oxidize less fat, when supplementing with exogenous ketones? I think you indicate in your article that it could. I would expect it to, in that if I supplement then my body would not “need” to oxidize the fat to provide the energy.

Wow! Amazing article. I don’t read blogs regularly, but I’ll be reading more of yours. I am on day 11 of a fast to get me into ketosis; 7 days bone broth and now on day-4 water only. Intentions are weight loss (10# down, 15# more to go) and immunity improvement/gut help. Planning 9 days of whey water, fermented veggies and mad probiotics when the water days are done (day 14?). Ketosis also might cause weight loss in the beginning of a diet because of a loss of water weight. Ketosis causes your body to get rid of sodium and to release more urine. This can result in losing about 100 to 150 calories a day, just from this aspect of ketosis. This is weight that will likely be regained, however. Many diets that result in ketosis, namely low-carbohydrate diets, are low in calories as well, which can lead to weight loss. Nonetheless, it is often an unhealthily low number of calories.

The ketogenic diet is a high-fat, adequate-protein, low-carbohydrate diet that in medicine is used primarily to treat difficult-to-control (refractory) epilepsy in children. The diet forces the body to burn fats rather than carbohydrates. Normally, the carbohydrates contained in food are converted into glucose, which is then transported around the body and is particularly important in fueling brain-function. However, if there is little carbohydrate in the diet, the liver converts fat into fatty acids and ketone bodies. The ketone bodies pass into the brain and replace glucose as an energy source. An elevated level of ketone bodies in the blood, a state known as ketosis, leads to a reduction in the frequency of epileptic seizures.[1] Almost half of children, and young people, with epilepsy who have tried some form of this diet saw the number of seizures drop by at least half, and the effect persists even after discontinuing the diet.[2] There is some evidence that adults with epilepsy may benefit from the diet, and that a less strict regimen, such as a modified Atkins diet, is similarly effective.[1] The most common adverse effect is constipation, affecting about 30% of patients—this was due to fluid restriction, which was once a feature of the diet, but this led to increased risk of kidney stones and is no longer considered beneficial.[2][3]

A high-fat diet also trains your body to burn even more fat during exercise, even at high intensities. It is interesting to note that the KB are capable of producing more energy than glucose due to the changes in mitochondrial ATP production induced by KB (Kashiwaya et al., 1994; Sato et al., 1995; Veech, 2004). During fasting or KD glycaemia, though reduced, remains within physiological levels (Seyfried and Mukherjee, 2005; Paoli et al., 2011). This euglycemic response to extreme conditions comes from two main sources: glucogenic amino acids and glycerol liberated via lysis from triglycerides (Vazquez and Kazi, 1994; Veldhorst et al., 2009). Glucogenic amino acids (neoglucogenesis from amino acids) are more important during the earlier phases of KD, while the glycerol becomes fundamental as the days go by. Thus, the glucose derived from glycerol (released from triglyceride hydrolysis) rises from 16% during a KD to 60% after a few days of complete fasting (Vazquez and Kazi, 1994). According to Bortz (1972) 38% of the new glucose formed from protein and glycerol is derived from glycerol in the lean while 79% in the obese (Bortz et al., 1972). It is important to note that during physiological ketosis (fast or very low calorie ketogenic diets) ketonemia reaches maximum levels of 7–8 mmol/L with no change in blood pH, while in uncontrolled diabetic ketoacidosis blood concentration of KBs can exceed 20 mmol/L with a consequent lowering of blood pH (Robinson and Williamson, 1980; Cahill, 2006) (Table ​(Table11).

I would then also hypothesize that certain people who feel that “brain fog” lifted on ketosis is due to either placebo effect OR their brain actually functioning differently on ketones vs glucose.

If you have poor liver or gallbladder function, it can be a good idea to take care of that prior to a high fat diet, yes. But a healthy high fat diet would not cause liver issues per se. Oxidation of cholesterol causes inflammation; not the other way around. So, your point about inflammation is a non-issue.
Urine strips or sticks: Ketone urine strips indicate ketone quantity on a color-grading scale. Urine tests are affordable and easy to use. However, be wary of inaccurate results. Urine tests only monitor leftover, unused ketones in your body. That’s why the strips will be darker, indicating more ketones, when you’re new to the diet. As you adapt to ketosis, your body uses more ketones, so there will be fewer ketones expelled through your urine. Being well-hydrated can also throw off tests.

The quantity of fat in the diet can be calculated from the overall energy requirements and the chosen ketogenic ratio. Next, the protein levels are set to allow for growth and body maintenance, and are around 1 g protein for each kg of body weight. Lastly, the amount of carbohydrate is set according to what allowance is left while maintaining the chosen ratio. Any carbohydrate in medications or supplements must be subtracted from this allowance. The total daily amount of fat, protein and carbohydrate is then evenly divided across the meals.[36]

I’m not sure if I really understand the idea of exogenous Ketones putting ones body into a state of KETOSIS, in that their own fat cells will be burned for fuel. Won’t the body just use the exogenous ketones and energy and continue to use glucose sources as well? How does taking ketones, make the body burn it’s own fat stores? I always thought that one would have to be in a state of energy deprivation for the body to actually begin to use the TG in the fat cells to generate ATP. Please let me know what I’m missing…

It’s in line with Dr. Atkins’s legacy; he was a huge proponent of alternative and unproven medical treatments beyond just his ideas about nutrition. You almost can’t blame him or the other diet gurus for leaning in on the techno-bullshit market; it’s hard to fill up a 300 page diet book on “eat a bit less and find a type of exercise that doesn’t make you hate life.”

But I did NOT love being constipated despite plenty of fibrous leafy greens and increased water consumption. As a nurse I know how important regular bowel function is and as I just couldn’t get around this one I have decided to loosen up a bit and follow the CSIRO low carb guidelines which allow a bit more carb, less fat and is way more bowel friendly. Hoping that it doesn’t affect those ghrelin levels too much.
