Thanks for the very informative article. This was the push I needed to at least try it. Starting next weekend, my partner and I will be trying keto for 30 days. I’m all for small changes, in theory, but what I sometimes discover is that sweeping changes can have their place, too, if they produce positive results. Often, what will work for me is trying a big change, and even if I then throw out 80% of it, at least the remaining 20% sticks. By contrast, any backsliding from a small change can often mean just throwing it out entirely.
So why did Atkins fall out of favor? Well, the diet’s delightful side effects could include fatigue, constipation, excessive thirst, bad breath, the dreaded meat sweats, and worst, the look your friends would give you when you said you were on Atkins. There was also the fact that the diet wasn’t really sustainable (kinda like most diets, funnily enough). In 2003, a pair of studies in the New England Journal of Medicine found that most of the weight one initially lost while following Atkins was water weight; and subjects who followed the diet typically gained back any lost weight in six months.
It is interesting to note that the KB are capable of producing more energy than glucose due to the changes in mitochondrial ATP production induced by KB (Kashiwaya et al., 1994; Sato et al., 1995; Veech, 2004). During fasting or KD glycaemia, though reduced, remains within physiological levels (Seyfried and Mukherjee, 2005; Paoli et al., 2011). This euglycemic response to extreme conditions comes from two main sources: glucogenic amino acids and glycerol liberated via lysis from triglycerides (Vazquez and Kazi, 1994; Veldhorst et al., 2009). Glucogenic amino acids (neoglucogenesis from amino acids) are more important during the earlier phases of KD, while the glycerol becomes fundamental as the days go by. Thus, the glucose derived from glycerol (released from triglyceride hydrolysis) rises from 16% during a KD to 60% after a few days of complete fasting (Vazquez and Kazi, 1994). According to Bortz (1972) 38% of the new glucose formed from protein and glycerol is derived from glycerol in the lean while 79% in the obese (Bortz et al., 1972). It is important to note that during physiological ketosis (fast or very low calorie ketogenic diets) ketonemia reaches maximum levels of 7–8 mmol/L with no change in blood pH, while in uncontrolled diabetic ketoacidosis blood concentration of KBs can exceed 20 mmol/L with a consequent lowering of blood pH (Robinson and Williamson, 1980; Cahill, 2006) (Table ​(Table11).

Take coconut oil for example. The coconut oil industry loves to market the idea that relatively inexpensive and abundant coconut oil is a great source of MCTs because it’s “62% MCT oil”, but the problem is that studies show you can’t get many useful ketogenic MCT’s from just eating coconut oil or even most brands of “MCT oil”, which are often is diluted with lauric acid, a cheap, hugely abundant part of coconut oil that is typically marketed as an MCT oil.
Don’t stop tracking your ketones when you’re trying to lose weight. An excellent way to keep your ketone levels up is taking exogenous ketones. More Sustained Energy: 90-120 minutes after you eat carbohydrates, your body doesn’t have readily available energy produced from the mitochondria in your cells, so you start “crashing” or lowering your energy. When you are in ketosis, your body can run off your body fat, which is an essentially limitless source of fuel. This prevents any type of crash.
In some cases epilepsy cannot be treated successfully using anticonvulsant medications. In some cases where drugs have failed, the ketogenic diet has been widely documented to deliver transformative seizure control, reducing frequency by anywhere between 40-90%43. If you give your body any more than the absolute minimum amount of protein that it needs, it will immediately break it down into carbs. This is why keto sites often give a guideline for not eating too much protein. The problem is that there’s no one guideline that works for everyone, and without specifically tailoring keto to your body it’d be easy to accidentally ingest too much protein.

Also, take special note of sugar alcohols like sorbitol, mannitol, xylitol, and isomalt. Sugar alcohols are derived from plant products (ie., fruits and berries), though their carbs are uniquely altered through chemical processing. While sugar alcohols provide fewer calories than table sugar, people often overeat them because they are labeled as “sugar-free” or “no sugar added” foods. This is problematic because they can still spike blood glucose levels due to their carb content. You still need to account for them in your diet plan.[2] Your best bet is to go with erythritol because it doesn’t cause blood sugar or insulin spikes.


Hi Ben, great article. I have been a keto-adapted athlete for over 2 years, all through nutrition (65/25/10). I have recently discovered UCAN, KetoOS and MAP Aminos. So, here’s my question: If I am going out for a 4-hour ride, and I want to fuel myself just on these supplements and my body’s natural fat stores, how and in what order should I take them? If I take them all together, will the aminos in the KetoOS interfere with MAP Aminos? Or should I just make a mix of the UCAN and KetoOS in 10oz of water and use it to wash back my 6 MAP tablets, 15-mins before my ride? Thanks for your advice!

No offense brother, but this whole article is a big selling advertisement. The health benefits of a “low – carb, high – fat” diet are well known and proven. I directly blame nutritionists, like yourself, for the fact that ~70% of American is considered obese now, not fat, obese. And the fact that Heart Disease is the number 2 killer in America now, second only to cancer.

I went into ketosis (using LOTS of coconut oil) but it raised my cholesterol and doc was worried about my lp(a) level (17mg/dL on VAP) even though crp was 0.5, trigs 51 and a1c was 5.2. I will begin a medically supervised weight loss program on Tuesday, that is intended to put me into ketosis via a very low calorie, high protein diet of shakes for two meals per day and one (controlled) regular meal. The overview of the program says to expect up to 2 weeks of foggyness and crankiness while getting in to ketosis. Will taking KetoCaNa 3 times a day for two days in advance of starting the diet (and during the introduction to the diet) help move me more quickly through the foggy, cranky phase? And should I also be eating (a ketogenic diet) during those two days or only drinking the KetoCaNa? My thanks in advance for any light you can shed on this!
He died in April this year due to a massive heart attack. He was only 38. Autopsy report showed severely advanced coronary artery disease and almost complete heart block. The medical examiner said that this had to of started in his early 20s to be this advanced and that it's very hereditary. (at that time he ate HORRID and smoked, was very very overweight, stressed to the max etc). She stressed that everyone in his family be checked including children.
If you’re looking to get a jump start on your health and fitness goals this year, you may be thinking about trying the ketogenic diet. Maybe you’ve heard the phrase before — it’s a huge diet buzzword — but aren’t sure what it means. Enter the Internet. I tried a bunch of things… ordered this powder and that “organic concoction,” etc… nothing… I’m the cook in the family and my love for food has reared its beautiful head again. So in the spirit of finding a balance and battling my own obsessive issues, I eventually found a tea online that’s touted as a “detox” tea but I wouldn’t really call it that. I would describe it as doing the same thing for my obsessions that, say, chamomile tea does for relaxation. It literally “relaxes” my obsessions an mental “food shackles.” Yes, everything about it feels “salesy,” but I’ve tried literally a ton of these things and this one has had an extremely positive effect on my cravings to the point where I finally feel like I’m in control of my “food brain” again. It is possible to lose weight in the state of ketosis, especially in the beginning. This state can reduce your feelings of hunger so you eat less, which is the result touted by low-carbohydrate diets. Ketosis might even make you nauseous, making you not want to eat. A May 2008 study in “The American Journal of Clinical Nutrition” points out, however, that not enough evidence backs up the idea that ketones suppress appetite.
As ketones are the only other metabolic substrate that can fuel the brain, there is a compelling mechanism whereby ketosis could improve brain energy metabolism and therefore improve symptoms of AD. Despite a declining ability of the brain to use glucose, cerebral ketone metabolism is preserved in AD (Castellano2015). This means that ketosis could be used to prevent an energy deficit in the brain. To test this, researchers developed a carbon-11 labeled AcAc (11)C-AcAc for PET use. They found in rats that after 10 days of KD (11)C-AcAc brain uptake increased up to 8-fold, an increase comparable to those measured after 48 h of fasting (Pifferi et al., 2008).

Question: I have recently done my 23&me DNA test and have also put my info into AthletiGen. I’m curious about your thoughts about high fat/low carb diet in context of this info. My test says I am ‘2x more likely to loose weight on a low fat diet’, and that I have ‘normal sensitivity to weight gain due to saturated fat intake/and unsaturated fat intake’, & ‘likely to eat normal amounts of carbs’! All interesting but in the ketosis context, I took this to mean that eating high fat/low carb would not be of greatest benefit to me?! (I have tried getting into ketosis prior to all the new supps, for about 3 months, and I couldn’t get in to it (tested w/ blood finger prick) and didn’t feel great). Which leads me to wonder what Macro ratio would be best, as I’m also a endo-meso)!
I’m with you. I have lost 45 pounds on Keto, and I’m now off ALL diabetic medication. Also the arthritis in my fingers and knees don’t hurt anymore. My energy is off the charts and I feel so good. I think people who have glycemic insulin resistance will live l9nger and may not lose limbs and need new livers because of high blood sugar. No diet or eating plan is for everyone, but for me Keto has saved my life and my feet. I’m so happy. Many Drs. Ketolysis is the process of breaking down ketones to ultimately provide energy through the Krebs Cycle and mitochondrial oxidative (using oxygen) phosphorylation. Ketone bodies are broken down in the mitochondria of virtually all tissues in the body. The liver is a notable exception, being unable to utilise ketones as a fuel because liver cells lack acetyl-CoA thiolase, a key enzyme in the ketone oxidative pathway. BHB enters the mitochondria of the cell through a monocarboxylate transporter, undergoes conversion to acetoacetate by BHB dehydrogenase and then addition of a CoA group from succinyl-CoA by 3-oxo-acid transferase. The resulting acetoacetyl-CoA acts a substrate for the formation of two molecules of acetyl-CoA in a reaction catalysed by acetyl-CoA thiolase. Acetyl-CoA is then available to condense with oxaloacetate and enter the Krebs cycle.
A common belief among keto devotees is that your body not only switches to burning fat on keto but that a low-carb, high-fat diet turns you into a “fat-burning machine.” Now that you’re not eating pizza crust, your body is going to burn through all of its own fat, calories and laws of thermodynamics be damned. I loved this post, thank you. I tried KETO for 1 day only and I felt terrible. It just wasn’t worth it to me to feel like shit even for one day and I knew I had to study a lot at the time, so I decided that keto is just not for me. Great post!! I’m a 41 year old Master CrossFit athlete, been in keto / LCHF Primal lifestyle for the past 9 years. I feel that my performance has improved a lot and continues to improve pretty significantly. I was a top 200 Master Open Qualifier on 2016 and 2017 and I’m usually on the podium of local competitions so my performance is really not bad…and I’m totally fat adapted. I follow sort of a TKD where I sometimes eat carbs at night during the week, but never above 100g so It doesn’t even kick me out of ketosis due to activity level. I’ve been playing with some measurements and I noticed that my BG reading after high intensity training sessions is really high (up to 180mg/dl). But it goes down fast (sometimes it goes down to 50mg/dl but I show no side effect of hypo, function completely normal). From my research, the high glucose post high intensity is normal and due to the stressful response of the exercise and also because my liver is producing the glucose from gluconeogeneses to provide it for the workouts, when needed. This only happens when the workouts are long, above 30 minutes.
Ketogenic diet could improve body composition: For some sports (such as gymnastics, cycling and some fighting sports), power to weight ratio is a key determinant of performance. Because the ketogenic diet has been associated with fat loss accompanied by lean muscle maintenance (or gain), it could thereby improve the power to weight ratio 21 ,22 and performance. 

I am new to Keto, but not new to a low carb diet. I just started a keto diet 8 days ago. My goals for being on the diet is a little body recomp (nothing major, a few pounds of fat loss), and moving to fat as primary fuel for endurance with a focus on trail ultra marathon races and training. Along with Keto I am doing heart rate training using MAF and OFM as a guide. supplemented with body weight and free weight strength training and some HITT.

If you’re following the keto diet, you will need protein, but you should limit your intake to about 20 percent of your total daily calories. (1) This is important because when you consume more protein than you need, your body converts the excess protein into carbs through a process called gluconeogenesis. This process pushes your body out of ketosis.
Children who discontinue the diet after achieving seizure freedom have about a 20% risk of seizures returning. The length of time until recurrence is highly variable but averages two years. This risk of recurrence compares with 10% for resective surgery (where part of the brain is removed) and 30–50% for anticonvulsant therapy. Of those that have a recurrence, just over half can regain freedom from seizures either with anticonvulsants or by returning to the ketogenic diet. Recurrence is more likely if, despite seizure freedom, an electroencephalogram (EEG) shows epileptiform spikes, which indicate epileptic activity in the brain but are below the level that will cause a seizure. Recurrence is also likely if an MRI scan shows focal abnormalities (for example, as in children with tuberous sclerosis). Such children may remain on the diet longer than average, and it has been suggested that children with tuberous sclerosis who achieve seizure freedom could remain on the ketogenic diet indefinitely.[45]

Fortunately, Drs. Kevin Hall and Juen Guo had the same burning question. In 2017, they analyzed the data and published a meta-analysis of controlled feeding studies that compared diets of equal calorie and protein content with variations in carbohydrate and fat content. [24] By filtering out the diet data in this way, the researchers could finally find out whether restricting carbs or limiting calories is more important when it comes to weight loss.
One downside to a ketogenic diet for weight loss is the difficulty maintaining it. “Studies show that weight loss results from being on a low-carb diet for more than 12 months tend to be the same as being on a normal, healthy diet,” says Mattinson. While you may be eating more satiating fats (like peanut butter, regular butter, or avocado), you’re also way more limited in what’s allowed on the diet, which can make everyday situations, like eating dinner with family or going out with friends, far more difficult. Because people often find it tough to sustain, it’s easy to rely on it as a short-term diet rather than a long-term lifestyle.
After about two to seven days of following this eating routine, you go into something called ketosis, or the state your body enters when it doesn't have enough carbs for your cells to use for energy. Type I diabetes is usually treated by insulin injections, that replace the body’s own insulin production. In Type I diabetics, lowering dietary carbohydrate consumption can reduce the need to inject insulin to lower blood sugar101. However, because they do not release any insulin Type I diabetics can be at risk of developing a complication called “Diabetic Ketoacidosis” (DKA). DKA occurs because, alongside its effects on glucose, insulin has other effects in the body. Insulin normally inhibits the release of fat (lipolysis) from adipose tissue. In Type I diabetics, the lack of insulin can lead to high levels of lypolysis, high levels of fatty acids in the blood, this then drives rapid and uncontrolled liver ketone production. The symptoms of DKA are weakness, confusion and deep gasping breathing. In order to avoid developing DKA while following a ketogenic diet, Type I diabetics should seek medical supervision and closely monitor their glucose and ketone levels if reducing their dietary carbohydrate intake. 
Thank you for this honest post on how Keto affected you. I did Keto for 6 weeks. I did lose 7 lbs but I struggled with depression and not using the bathroom regularly nice I started the diet. I searched and found carbs help with serotonin uptake which explained my depression. Everywhere I look people are pro Keto instead of discussing some the side effects. My gym friend told me she cried the last 4 days she did Keto before saying enough is enough. If it works for others, that’s great, but it’s refreshing to hear that I am not the only one who struggled and stopped Keto.

In the 1960s, it was discovered that medium-chain triglycerides (MCTs) produce more ketone bodies per unit of energy than normal dietary fats (which are mostly long-chain triglycerides).[15] MCTs are more efficiently absorbed and are rapidly transported to the liver via the hepatic portal system rather than the lymphatic system.[16] The severe carbohydrate restrictions of the classic ketogenic diet made it difficult for parents to produce palatable meals that their children would tolerate. In 1971, Peter Huttenlocher devised a ketogenic diet where about 60% of the calories came from the MCT oil, and this allowed more protein and up to three times as much carbohydrate as the classic ketogenic diet. The oil was mixed with at least twice its volume of skimmed milk, chilled, and sipped during the meal or incorporated into food. He tested it on twelve children and adolescents with intractable seizures. Most children improved in both seizure control and alertness, results that were similar to the classic ketogenic diet. Gastrointestinal upset was a problem, which led one patient to abandon the diet, but meals were easier to prepare and better accepted by the children.[15] The MCT diet replaced the classic ketogenic diet in many hospitals, though some devised diets that were a combination of the two.[10]

While it also has been tried for weight loss, only short-term results have been studied, and the results have been mixed. We don't know if it works in the long term, nor whether it's safe," warns registered dietitian Kathy McManus, director of the Department of Nutrition at Harvard-affiliated Brigham and Women's Hospital.

The low glycaemic index treatment (LGIT)[48] is an attempt to achieve the stable blood glucose levels seen in children on the classic ketogenic diet while using a much less restrictive regimen. The hypothesis is that stable blood glucose may be one of the mechanisms of action involved in the ketogenic diet,[9] which occurs because the absorption of the limited carbohydrates is slowed by the high fat content.[5] Although it is also a high-fat diet (with approximately 60% calories from fat),[5] the LGIT allows more carbohydrate than either the classic ketogenic diet or the modified Atkins diet, approximately 40–60 g per day.[3] However, the types of carbohydrates consumed are restricted to those that have a glycaemic index lower than 50. Like the modified Atkins diet, the LGIT is initiated and maintained at outpatient clinics and does not require precise weighing of food or intensive dietitian support. Both are offered at most centres that run ketogenic diet programmes, and in some centres they are often the primary dietary therapy for adolescents.[9]


Keto meals seemed so heavy and rich to me, and it just wasn't how I was used to eating. Plus, I consider myself a flexitarian, so the thought of eating more meat—and more often than I typically would—gave me pause. Dr. Axe assured me that even vegetarians could pull off a keto diet if they planned well enough. (Vegans can, too.) That said, he's a proponent of animal products and red meat specifically, because of the iron it provides for women (who are more susceptible to deficiency) and because it can bolster energy when carbs are lacking. The brain is different as it is dependent on carbohydrates as a fuel source. This is because fats cannot easily cross the blood-brain barrier. The inability to make use of energy within fat poses a problem during periods where there is limited carbohydrate in the diet. If blood glucose levels fall to low, brain function declines. Relatively little energy is stored as carbohydrate (2,000 kCal) compared to fat (150,000 kCal). The body's store of carbohydrates runs out with a few days of carbohydrate restriction. Once glycogen is depleted, a cascade of hormonal signals causes the body to increase the release of stored fats (from adipose tissue). Signals include the fall in blood insulin, rise in a hormone called glucagon and an increase in cortisol (stress hormone) 1. The increase in blood fatty acids is a key trigger for ketone production (ketogenesis). Unlike fats, ketones are readily used as a fuel in the brain. Fatty acids are converted into ketone bodies in the liver, and ketones can provide up to 60% of the brain's energy requirements during starvation 2. The graph below shows how BHB (black triangles) builds up in the blood over many days until it reaches a level of around 6 mM.
Around the same time, it was discovered that the metabolic change caused by fasting that controlled seizures also occurred when a patient stopped eating carbohydrates. It was then that an endocrinologist named Dr. Rollin Woodyatt had a scientific breakthrough: he found that the compounds acetone and beta-hydroxybutyric acid were detectable in high levels in fasting patients (to be fair, they can also be present in urine in low levels normally, and things like dehydration can trigger a false positive test). These compounds are classified as ketones, produced via the metabolic state of ketosis, which occurs when an elevated level of ketones are produced as a result of the body using fat for fuel as stored carbohydrate is depleted.
Move more. You’ll lose pounds faster if you increase your daily physical activity. Keep in mind you don’t have to go to the gym 6 times a week or jog every morning, just move more in your everyday life. For instance, take a short 2-minute break from sitting in your chair every hour, take the stairs instead of the elevator, walk to make errands if you can, get a standing desk, or take phone calls standing up and pacing around. These small calorie-burning movements add up at the end of the day.
