This is a great post and very informative. I have been following a keto diet now for almost 3 mths. I have only lost 8 pounds but lost all my bloating and lost lots of inches. I have severe IBS and fear eating anything outside of my own home and bathroom. Since on keto diet that has all gone away and I have no fear anymore!!! I know keto isn’t for everyone and I want to lose more weight but I will give up the weight loss to have my IBS corrected any day!!

When a ketone ester drink was taken pre-workout, the muscle used far less glycogen compared to when the pre-workout drink contained carbohydrate. The high levels of blood ketones meant that the muscle used ketones as a fuel before needing to use glycogen. Reducing reliance on muscle glycogen could improve performance and decrease the time for muscle glycogen to fully recover between exercise bouts.

Another mechanism that could be involved in food-regulation during KD is the gamma aminobutyric acid (GABA) and glutamate regulation. Wu et al. demonstrated that GABAergic signaling from the NPY/AgRP neurons to the parabrachial nucleus (located in the dorsolateral part of the pons) is involved in many regulatory sensory stimuli including taste and gastric distension, regulate feeding behavior. GABA signaling seems to prevent animals from anorexia when AgRP neurons were destroyed (Wu et al., 2009). These findings are yet another contradictory aspect of KDs and food behavior; ketosis should increase the availability of glutamate (via diminution of transamination of glutamate to aspartate) and therefore increase GABA and glutamine levels; moreover, in ketosis, the brain imports a huge amount of acetate and converts it through glia into glutamine (an important precursor of GABA) (Yudkoff et al., 2008). The result of these mechanisms, together with the increased mitochondrial metabolism and flux through the TCA cycle, is an increased synthesis of glutamine and a “buffering” of glutamate. These results are not consistent with the well-documented anorexigenic effect of KDs, and therefore the GABA hypothesis cannot be taken into account despite the mild euphoria often reported during a KD that is probably due to the action of BHB (Brown, 2007) and can help to reduce appetite.
“To achieve the relatively high rates of intake (up to 90 grams/hour) needed to optimize results in events lasting longer than three hours, athletes should practice consuming carbohydrates during training to develop an individual strategy, and should make use of sport foods and drinks containing carbohydrate combinations that will maximize absorption from the gut and minimize gastrointestinal disturbances.”
-Nervous System Damage: It’s been shown that patients with neuropathy whose after-meal glucose readings were above the diabetic threshold sustained damage to their large nerve fibers. Even neuropathy patients whose glucose readings remained well within the normal range showed damage to their small nerve fibers. Studies have shown that within any blood sugar range, the higher the glucose, the greater the damage to nerve fibers.

Thank you for sharing, my husband and i have switched to the keto diet and we love it, we don’t feel restricted at all when it comes to meal planning, i actually feel like we eat more adventurous because we’re always looking for more receipts on how to prepare food. I’ve always done the same dinners, meat, veggies and a rice or pasta side, but with this diet we get to experiment, each night is something new to try. The Ketogenic diet is not about cutting all carbs, just cutting way back on carbs. We have gone and had regular check ups at the doctor with no negative results. I for one have always had an issue with poor digestion but since switching to the keto diet that has drastically changed. The hardest part for me was to cut pasta but i found great joy and fun in finding alternatives to pasta so i can still enjoy the same dish minus the carbs. There are so many different diets out there and so many different types of people so you have to find the right one for you, the keto diet is not for everyone but for us, so far it seems beyond perfect. Weight loss caused by low-carb diets isn’t just because of a loss of “water weight”. In research studies that measured change in body fat, subjects eating low-carb diets had a greater loss in body fat than those eating low-fat diets. Additionally, keto diets lead to a greater reduction in waist circumference, a critical indicator of harmful stomach fat.
The ketogenic diet can compromise high intensity sprint performance: High intensity exercise performance is heavily reliant on the ability to produce energy via anaerobic respiration (glycolysis), which requires carbohydrate as a substrate. Following a ketogenic diet causes a decrease in the amount/activity of the enzymes in the glycolysis pathway that decreases the rate that the pathway can proceed24. This could explain the decrease in anaerobic sprint performance consistently seen with athletes following a ketogenic diet 25 ,23. 

This is because you get rid of the foods that cause high blood sugar, also known as glucose. Glucose is the easiest way for your body to get energy, but it can lead to energy crashes if you get too much in a short time. To get rid of the glucose and use it up, the body needs to produce insulin. As we produce more and more, our bodies become resistant to it, and this can lead to diabetes.
The keto diet isn’t new, and it’s been around for nearly a century. It was originally developed to treat people with epilepsy. In the 1920s, researchers found that raised levels of ketones in the blood led to fewer epileptic seizures in patients. The keto diet is still used today to treat children with epilepsy who don’t respond well to anti-epileptic drugs.[2]

I’m not sure if I really understand the idea of exogenous Ketones putting ones body into a state of KETOSIS, in that their own fat cells will be burned for fuel. Won’t the body just use the exogenous ketones and energy and continue to use glucose sources as well? How does taking ketones, make the body burn it’s own fat stores? I always thought that one would have to be in a state of energy deprivation for the body to actually begin to use the TG in the fat cells to generate ATP. Please let me know what I’m missing…

When glucose levels are low, especially over time, most cells will switch to using ketone bodies for fuel. Ketones allow cells to be metabolically flexible, so to speak. Even the brain and nerve cells, which are heavily dependent on glucose can utilize ketone bodies for fuel. This ability of most normal cells to use ketones when glucose is unavailable indicates that their cellular mitochondria are healthy and functioning properly. 
I appreciate you sharing your story. In this day and age Keto is the holy grail for some. There are those of us though that do not respond well. I’m one of them. I couldn’t lose weight, I didn’t feel good (and that was after the Keto “flu”) and my periods were coming every 2 weeks (I’m 48 and peri-menopausal). What I did find, after the same soul searching, was that I do have some sort of weird physical reaction to wheat in any form. If I avoid it, I feel better. Whatever works, we’re all unique and what works for one will not work for all. That’s my big take away. I can appreciate that people have had success but something about going THAT low in my carbs created a stress response in my body. Also, I was needing to use my emergency Xanax a whole lot more. This is the second shortest form of MCT, also rare and comprising about 8% of coconut oil. It is slower to turn into energy but less expensive than C8. XCT Oil is triple-distilled in a non-oxygen atmosphere with no solvents, and it contains C10 and C8, because these are the only two MCT oils that turn into ATP quickly without the liver. You would need 6 tablespoons of coconut oil to get one tablespoon of XCT oil.
The problem with this is that MOST people don’t turn to keto as a lifestyle. Many turn to it and use it as a diet for quick weight loss. If you love keto and live a keto lifestyle, not feeling deprived in any way, then okay, eat keto! But I don’t agree that we need to demonize certain foods. This can lead to eating disorders and an unhealthy relationship with food, and this is why I wrote this article, to help people like me who have to deal with this.

The 2017 meta-analysis by Drs. Kevin Hall and Juen Guo provide us with very convincing data, but we must also consider the fact that the data came from studies where all the food was provided by the scientists. Although this is a great way to assess the difference between low-carb and high-carb diet, this does not simulate the real-world effectiveness of each diet. For this reason, we must investigate data from less strict studies. In other words, we need to look at what happened when subjects were told to follow a specific diet on their own.
The point of keto is to force your body to deplete its glucose (and the stored form, glycogen) so it will have to use body fat as a fuel source. It’s capable of making ketone bodies from your fat, which can replace glucose as an energy-storing molecule if necessary. To do that, you have to break apart fat molecules thus ‘burning’ the fat off. But here’s the thing: your body really really doesn’t want to run out of glucose. No glucose means starvation as far as it’s concerned—even if you're not feeling hungry, your body is still missing one of its key macronutrients. And when you’re (nutritionally) starving, your body will start to break down protein just to get those sweet, sweet carbs. Of course, you have a source of protein in your body already: your own muscles. “When in starvation mode, your body breaks down muscle in your body,” says Giancoli. “Ketosis is a way of trying to preserve that protein. It’s not ideal, but it’s your body’s way of saving you.”
The concentration of ketone bodies may vary depending on diet, exercise, degree of metabolic adaptation and genetic factors. Ketosis can be induced when a ketogenic diet is followed for more than 3 days.[34] This induced ketosis is sometimes called nutritional ketosis.[35] This table shows the concentrations typically seen under different conditions[1]
The ARC exerts opposing actions on food intake responding not only to leptin and insulin, but also to gut hormones (the most studied are ghrelin and, recently, PYY). The neurophysiological pathways suggest that feeding is regulated by a feedback loop, where the hypothalamus provides the long-term regulatory input to the NTS, which acts as a setpoint (Williams et al., 2001).
One hypothesised contributor to neuronal death is insufficient energy production, secondary to impaired mitochondrial function. However, it is unclear if this is in fact a cause or effect of PD. Whatever the case may be, patients with PD have been shown to have impaired mitochondrial energy production in the brain59 and lower brain glucose utilisation60. Another factor may be neuro-inflammation, which is also common in PD, and is thought to lead to further accumulation of Lewy Bodies and neuronal death.
Your dietary needs change dramatically with keto. Sodium becomes a critical nutrient, as do magnesium and potassium. “At least in the initial stages of being on the keto diet, you will urinate more. One of the key electrolytes lost through urination is magnesium,” says Carolyn Dean, MD, ND, a health and nutrition expert and author of The Magnesium Miracle. “Magnesium is an energy mineral, which will help you burn fat and lose weight.”
Curiously, this study was funded by the Nutrition Science Initiative (NuSI), a group with the aim of producing “conclusive results in the next decade” in a sometimes confusing nutritional landscape. They claim our nutritional guidelines are “based on inconclusive science,” and though their website doesn’t directly indicate any bias, their research so far focuses on the effects of carbohydrates on obesity. This was the second published study that received funding from the institute. In the first study, published in the American Journal of Clinical Nutrition in 2016, researchers hypothesized that a low-carbohydrate diet increased energy expenditure. Results said otherwise:
If you have a functioning pancreas that can produce insulin – i.e. you don’t have type 1 diabetes – it would be extremely hard or, most likely, impossible to get ketoacidosis even if you tried. That’s because high ketone levels result in release of insulin, that shuts down further ketone production. In other words, the body has a safety net that normally makes it impossible for healthy people to get ketoacidosis.
Longer-term ketosis may result from fasting or staying on a low-carbohydrate diet (ketogenic diet), and deliberately induced ketosis serves as a medical intervention for various conditions, such as intractable epilepsy, and the various types of diabetes.[6] In glycolysis, higher levels of insulin promote storage of body fat and block release of fat from adipose tissues, while in ketosis, fat reserves are readily released and consumed.[5][7] For this reason, ketosis is sometimes referred to as the body's "fat burning" mode.[8]

It’s really rich and I enjoy it sprinkled with some New Zealand sea salt crystals. The crunch and saltiness of the crystals really complements the richness and nuttiness of the yogurt. I originally started this “diet” because I am the Maid of Honor in my best friends wedding. She made me order a size down than I was comfortable with and it fit, if you didn’t zip it up. I tried it on a couple weeks later and it REALLY didn’t fit, I couldn’t pull it up past my hips. I was mortified and super embarrassed, how was I going to be in this wedding if I can’t even fit in this dress. Cut to now, I just tried on my dress and it’s too big! I’m so excited that I have to get it altered.
Although convincing, the bulk of evidence in relation to the inhibitory effects of ketosis on appetite is still anecdotal. Preliminary scientific reports seem to support this phenomenon, and the evidence shows that KD is more effective, at least in the short/medium-term, on fat loss (Paoli, 2014). It was demonstrated that diet-induced weight loss leads to changes in energy expenditure and in appetite-regulating hormones that facilitate weight regain and the return to initial energy homeostasis (Sumithran et al., 2011). This response to alteration of energy balance nullifies the success of many dietary approaches. It is well-known that the long-term success of a nutritional approach is defined by the amount of weight regain and is the main problem regarding the so-called weight cycling or “yo-yo” effect (Jeffery, 1996). A recent study by our group has demonstrated that a brief ketogenic period, if followed by a longer period of correct Mediterranean diet could avoid this yo-yo effect (Paoli et al., 2013). During the ketogenic period subjects reported less hunger, confirming previous studies (Nickols-Richardson et al., 2005; Johnston et al., 2006; Johnstone et al., 2008) on hunger-suppression effect of ketogenic diet. Despite these clinical findings, the mechanisms of action of ketosis on appetite reduction are still not completely understood. Clinical results are suggestive of both direct and indirect (via modifications of hunger-related hormones concentration) actions of KBs on appetite (Sumithran et al., 2013).

Great post!! I’m a 41 year old Master CrossFit athlete, been in keto / LCHF Primal lifestyle for the past 9 years. I feel that my performance has improved a lot and continues to improve pretty significantly. I was a top 200 Master Open Qualifier on 2016 and 2017 and I’m usually on the podium of local competitions so my performance is really not bad…and I’m totally fat adapted. I follow sort of a TKD where I sometimes eat carbs at night during the week, but never above 100g so It doesn’t even kick me out of ketosis due to activity level. I’ve been playing with some measurements and I noticed that my BG reading after high intensity training sessions is really high (up to 180mg/dl). But it goes down fast (sometimes it goes down to 50mg/dl but I show no side effect of hypo, function completely normal). From my research, the high glucose post high intensity is normal and due to the stressful response of the exercise and also because my liver is producing the glucose from gluconeogeneses to provide it for the workouts, when needed. This only happens when the workouts are long, above 30 minutes.
Thanks for your comment. When I was reading this and you said. “I’ve been around this size all of my life” that says to me you are near your set point weight – the weight your body likes to be at. Is that maybe heavier than you would like? Perhaps. But does it mean you’re unhealthy? Certainly NOT!! Why do you feel the need to lose 50lbs? Is this a number you’ve made up in your head? A number you think you would feel better about yourself at?
A lot of bad information has been put out there due to an extremely large group of people selling exogenous ketones. They were not educated on a proper keto diet or the science behind it and are telling people if you’re not losing weight eat more fat. This I know because I was one of them being told that. I did not lose weight and got tired of eating all that fat. When macros are calculated whatever the fat amount comes out to is meant to be a limit not a goal to meet. Protein is the goal that needs to be met to not lose muscle mass. Fat should not account for 80% of a persons daily intake. If you look up keto books and information from Maria Emmerich you will be correctly informed.
This makes it easy to eat less and lose excess weight – just don’t eat until you’re hungry. This makes intermittent fasting easy, super-charging efforts to reverse type 2 diabetes and speeding up weight loss. Plus, you’ll save tons of time and money by not having to snack all the time. Many people only feel the need to eat twice a day (often skipping breakfast), and some just once a day.
Day 10: I'm starting to get sick of the same foods that I know are safe bets. And the number of times I've Googled: "Is _____ keto?" is getting out of hand. I've realized that the only real gripe I have with the keto diet is that there are so many healthy, nutritious foods that you can't eat while on it.
