In Alzheimer’s disease (AD), the function of the brain is compromised by the buildup of debris (plaques and tangles) inside the neurons. This mainly occurs in the areas of the brain associated with memory, intelligence, judgement, behaviour and language and impairs the ability to complete normal day to day tasks and to interact socially. Whilst the symptoms of AD usually only begin to appear with age, evidence suggests that damage to the brain begins to accumulate years earlier. This includes the buildup of plaques and tangles and a decreased ability to metabolise glucose (brain insulin insensitivity)50. If an individual has Type 2 Diabetes (systemic insulin insensitivity), the risk of AD is tenfold higher51.

Checking your ketone level is one way to know if you’re in ketosis. This metabolic state usually kicks in after three or four days of restricting your carbohydrate intake or going through periods of intermittent fasting. You don’t have to visit a doctor to measure your level. Pick up a ketone urine test from a nearby drug store, or use a blood sugar meter that’s capable of measuring ketones.

I'm not usually a fan of prescribed "diets"—I live by more of the anti-diet mantra. Call me jaded, but the word "diet" has a bit of a negative connotation these days, with "fad" and "restrictive" usually preceding it. It is possible to lose weight in the state of ketosis, especially in the beginning. A May 2008 study in “The American Journal of Clinical Nutrition” points out, however, that not enough evidence backs up the idea that ketones suppress appetite.
Selecting the right food will be easier as you become accustomed to the Keto approach. Instead of lean meats, you’ll focus on skin-on poultry, fattier parts like chicken thighs, rib-eye steaks, grass-fed ground beef, fattier fish like salmon, beef brisket or pork shoulder, and bacon. Leafy greens such as spinach, kale and lettuce, along with broccoli, cauliflower and cucumbers, make healthy vegetable choices (but you’ll avoid starchy root foods like carrots, potatoes, turnips and parsnips). I’m a former low-carber, and while I did feel great while on keto, and it did help me to lose the biggest chunk of weight I have lost, I ended up finding that it was not a sustainable approach for me. If I ever slipped up, the bloating and weight gain that would happen overnight was so demoralizing (even if it was just water weight fluctuating up) that I would go into an emotional downward spiral complete with a crazy carb bender and gain everything back for real in a matter of weeks. I remember that at one point I was actually using test strips, and I got kicked out of ketosis by a meal of salmon and Brussel sprouts that had a sugary glaze on them, which I had chosen as a healthier option at a restaurant while my husband and my friends ate pizza. I was so angry that that had happened when I had been trying so hard to be good and I went right “back on the wagon” the next day, but I could not get back into ketosis, and I stopped losing weight. A consistent healthy pattern should not be so hard to maintain. I would tell people all day and all night that I didn’t feel deprived, and I didn’t think I was lying, but I think there was evidence in my behavior that I did indeed feel deprived.
Great article! I tried Keto for a while & became a slave to food. I didn’t stick with it long enough to notice any effects on my health or energy, but the fact that all I could think about was what I could eat next and when, was enough for me to call it quits. I am in the military, and need to stay in top shape and have lots of energy, so my diet contains lots of protein, moderate fats, & moderate to high-carbs. I agree with the Keto advocates that fat is not bad, refined sugar has little to no nutritional value, and the Standard American Diet will probably kill us, but eating legumes and whole grains will not. Like you say, Keto might work for some, but it doesn’t seem practical for most of us. Thank you for your story and a common sense article.

This is because you get rid of the foods that cause high blood sugar, also known as glucose. Glucose is the easiest way for your body to get energy, but it can lead to energy crashes if you get too much in a short time. To get rid of the glucose and use it up, the body needs to produce insulin. As we produce more and more, our bodies become resistant to it, and this can lead to diabetes.
Fat: Most of the calories in a ketogenic diet come from fat, which is used for energy. The exact amount of fat a person needs to eat will depend on carbohydrate and protein intake, how many calories they use during the day, and whether they are losing weight (using their body fat for energy). Depending on these factors, somewhere in the range of 60 to 80 percent of calories will come from fats on a ketogenic diet (even up to 90 percent on, for example, the Ketogenic Diet for Epilepsy). People tend not to overeat on diets this high in fat, so calorie counting is rarely necessary.
Around the same time, it was discovered that the metabolic change caused by fasting that controlled seizures also occurred when a patient stopped eating carbohydrates. It was then that an endocrinologist named Dr. Rollin Woodyatt had a scientific breakthrough: he found that the compounds acetone and beta-hydroxybutyric acid were detectable in high levels in fasting patients (to be fair, they can also be present in urine in low levels normally, and things like dehydration can trigger a false positive test). On Cyclical keto, I noticed these weird bouts of depression every time I carbed up on the weekends. The hypothalamus is the brain's main center responsible for hunger/satiety (H/S) control. In the theory that Mayer proposed more than 60 years ago, he assigned a central role to glucose levels in the H/S control: the so-called “glucostatic theory” (Mayer, 1955). Mayer suggested that depletion of carbohydrate availability leads to hunger, and the hypothalamic centers with receptors sensitive to glucose levels might be involved in the short-term regulation of energy intake (Mayer, 1955). The “feeding center” in the lateral hypothalamic area (LHA), according to the glucostatic theory, reacts to the between-meal fall of blood glucose and stimulates food intake. The LHA contains glucose-inhibited neurons that are stimulated by hypoglycemia, a process crucial to mediating the hyperphagia normally induced by hypoglycemia. The subsequent post-prandial hyperglycemia activates the “satiety center” in the ventromedial hypothalamus (VMH), which contains glucose-excited neurons and inhibits both “feeding center” and food intake.
I am not a doctor and this is not to be taken, interpreted or construed as medical advice. If you have poor liver or gallbladder function, it can be a good idea to take care of that prior to a high fat diet, yes. But a healthy high fat diet would not cause liver issues per se. These are just my own personal thoughts and not a prescription or a diagnosis or any form of health care whatsoever.
I am curious if someone takes any or too much MCT oil (5-10 tablespoons/day) or Exogenous Ketones will the liver slow/stall or shut off endogenous ketone production as you are providing the ketones exogenously and thereby you may NEED to supplement ketones to maintain ketone levels or you may suffer a short term ketone deficit while the liver adjusts to making them on its own again. A parallel I am thinking of is exogenous testosterone supplementation and the hypothalamus/endocrine system slowing/stopping endogenous testosterone production (seen in bodybuilders).
Thanks for your comment. First off, I’m not influencing anyone to not try something. What I’m doing is giving people a safe space to say, “hey, this didn’t work for me, and that’s okay!” You have to remember, I’m writing this from MY perspective and how keto turned my eating into a disordered eating pattern. Yes, there are resources out there people can find to support keto, but there are also resources out there that show that keto isn’t for the average person. We can’t just look at one side, and that’s the conversation I’m trying to have.

My son had dropped slight hints over the previous year that he was concerned about me being obese and what that could do to my health. I never really paid attention to him but in the back of my mind I knew he was correct. My retirement hobbies were seriously studying cooking indoors and outdoors... plus making Artisan sourdough bread. Plus I have a wine cellar that has enough inventory that restricts me from buying more. None of which is conducive to losing 106 pounds.

Protein: Keep in mind that keto is high-fat, and not high-protein, so you don’t need to eat very much meat. Too much protein turns into glucose in the body, making it harder to stay in ketosis. Stick to fatty cuts of grass-fed, pasture-raised, or wild meat, and wild-caught fish. Red meats, offal/organ meats, pork, eggs (preferably pastured), fish, shellfish, and whey protein concentrate.

I totally agree with what you just wrote. I also follow Maria and Craig Emmerich. Science based, well regulated and truly a Ketogenic way of eating. So much misinformation on so many of the low carb websites and no science to back up their information. Three years Keto and didn’t do it for weight loss, I’ve always been thin and ate what is referred to as a healthy low fat diet till my insulin began sky rocketing into pre diabetes. This is not a diet, this a lifestyle. Calories in and calories out still mater. I count total carbs and stay at 25-30 total not net. If your trying to lose weight then fat is lever not a goal, you want to burn your own body fat, fat to feel full, that is all. Cheese and nuts are inflammatory for some and you will not lose weight till the inflammation is gone. High fat (only for those who have reached their goal weight), moderate protein (60-80 is the normal range for women) , low carb (under 30 total gram a day). I have never felt better or been healthier. Maybe not for everybody, perfect for me. Keto for life

Unfortunately, our current food environment is nothing like what the human race initially evolved to handle. Look into cold showers and ice baths for relief from depression along with all the other benefits it provides. If you’re willing to push your comfort zone, you’ll find that cold showers/baths provide a monumental difference. There’s some research out there that identifies the benefits of combining specific ketogenic diets and ice baths. And I used this to increase gains in the gym, reduce inflammation, feel better in general, builds patience and raises tolerance levels. It’s impressive stuff. Cold and heat thermogenesis for the win (heat would be sauna/steam room, but cold is what helps you sleep better and will help you with depression). Good luck mate!
Also, when I read what you’re eating, that doesn’t seem like nearly enough!! I’m not one to promote counting calories, however, I do feel that it can be helpful to see if you are eating enough. That doesn’t sound like it’s even close to 2,500-3,000 calories. Yes, you read that right…2,500 – 3,000 calories!!! When we don’t eat enough, our metabolism slows and no matter what we do, we won’t lose weight.

I’m honestly a little skeptical about it the idea of keto permanently. My brief glance at the literature seems to imply that it can have side effects of kidney stones, skeletal fractures, and slow the growth rate of children, but that was a study on kids with epilepsy (which it treated very effectively), so who knows how that applies to adults. And the other studies I found dealt with overweight and obese subjects, so it may be hard to find something on the long term effects on otherwise healthy adults.

Still, I enjoy cooking and I plan my meals on the regular anyway. With a keto food list in hand and advice from Dr. Axe in my mind, I filled my grocery cart with family-size versions of what I regularly buy (apples, berries, nut butter, kale), and a lot more meat than I ever have in my cart at one time (ground lamb, chicken, REAL bacon). What was missing? Some of my usual high-carb items, like whole-grain English muffins, orange juice, butternut squash, and tortilla chips.
I’m missing one key-step in this whole process … I understand (vaguely) how to get into ketosis via diet, starvation, or supplementation. But, my confusion really is with the supplementation method (such as XCT Oil, for example). If I supplement with these to get into ketosis quicker, but haven’t significantly altered to a high-fat diet, won’t I just shift back into glucose-use as soon as I burn out the supplemented ketones?? like within minutes / hours?
The discovery of many appetite-related hormones provided molecular basis for appetite control, decreasing the relevance of the metabolites hypothesis (Karatsoreos et al., 2013). Recently, Sumithran et al. demonstrated that there is a long-term persistence of changes in some peripheral hormones involved in food control (Sumithran et al., 2011). In this study, they found a significant difference in mean levels of many food intake-related hormones 1 year after the cessation of weight loss via the hypocaloric diet. There was a long lasting decrease of anorexigenic compounds: leptin, PYY, cholecystokinin, insulin, and pancreatic peptide and an increase of the orexigenic molecule ghrelin. Moreover, they found that hunger remained elevated 1 year after diet cessation. In a successive study the same group investigated hunger-related hormones after 8 weeks of KD, demonstrating that during ketosis the increase of ghrelin (a strong stimulator of appetite) was suppressed (Sumithran et al., 2013). These results are consistent with those of Ratliff et al (Ratliff et al., 2009), who found no significant change in fasting plasma ghrelin after 12 weeks of VLCD.

In the first week, many people report headaches, mental fogginess, dizziness, and aggravation. Most of the time, this is the result of your electrolytes being flushed out, as ketosis has a diuretic effect. How It Works On cyclical keto, also called keto cycling, you’ll cycle in and out of keto — usually on the diet for five days, followed by one or two days with more carbs. “The point of keto cycling is to make it easier for someone to follow,” Kizer says. “Every five to six days they can have the carbohydrates they’ve been entirely restricting.” There’s no set protocol of what your carb days should look like, but Kizer warns not to go overboard because that will make it more difficult for the body to return to ketosis.
With the recent research findings, and the increasing availability of exogenous ketones, it is unsurprising that some authors have said (with a hint of skepticism) that they “could be the next magic bullet’ for athletes39. More research is required to understand the best use cases, doseage protocols, compounds etc, however it is clear that exogenous ketones are a new ‘tool’ in the athlete’s arsenal that can be used to provide an alternative, energetically favourable fuel source without needing dietary manipulation. 

Some athletes swear by the ketogenic diet, not just for weight loss but for improved performance in their sport, as well. But Edward Weiss, PhD, associate professor of nutrition and dietetics at Saint Louis University, doesn’t buy it. “I hear cyclists say all the time that they’re faster and better now that they’re on keto, and my first question is, 'Well, how much weight did you lose?'” he says.

The discovery of many appetite-related hormones provided molecular basis for appetite control, decreasing the relevance of the metabolites hypothesis (Karatsoreos et al., 2013). Recently, Sumithran et al. demonstrated that there is a long-term persistence of changes in some peripheral hormones involved in food control (Sumithran et al., 2011). In this study, they found a significant difference in mean levels of many food intake-related hormones 1 year after the cessation of weight loss via the hypocaloric diet. There was a long lasting decrease of anorexigenic compounds: leptin, PYY, cholecystokinin, insulin, and pancreatic peptide and an increase of the orexigenic molecule ghrelin. Moreover, they found that hunger remained elevated 1 year after diet cessation. In a successive study the same group investigated hunger-related hormones after 8 weeks of KD, demonstrating that during ketosis the increase of ghrelin (a strong stimulator of appetite) was suppressed (Sumithran et al., 2013). These results are consistent with those of Ratliff et al (Ratliff et al., 2009), who found no significant change in fasting plasma ghrelin after 12 weeks of VLCD.

You indicate that exogenous ketones do not shut down the ability, of your body, to oxidize fat. Is that to say it does not have an effect on your body at all? My specific question is… does my body oxidize less fat, when supplementing with exogenous ketones? I think you indicate in your article that it could. I would expect it to, in that if I supplement then my body would not “need” to oxidize the fat to provide the energy.
Someone trying to follow the keto diet to a letter may not even go into the much-desired state of ketosis. Sure, you can buy those little piss strips that react to ketones in your urine to reassure you that you’re “in ketosis,” but you’re probably wasting your money. Ketones are present in low levels in your urine even if you’re not on a ketogenic diet. Measuring ketones via blood is far more accurate (if not overly invasive for a diet), but to measure them for weight loss in the first place is borderline useless, especially in urine.
There are three instances where there’s research to back up a ketogenic diet, including to help control type 2 diabetes, as part of epilepsy treatment, or for weight loss, says Mattinson. “In terms of diabetes, there is some promising research showing that the ketogenic diet may improve glycemic control. It may cause a reduction in A1C — a key test for diabetes that measures a person’s average blood sugar control over two to three months — something that may help you reduce medication use,” she says.
