KBs can cross the BBB but not in a homogenous manner. For example, past experiments have demonstrated that BHB utilization is different in various brain areas (Hawkins and Biebuyck, 1979). Areas without BBB, hypothalamic regions and the lower cortical layers have a higher BHB metabolism compared to the lower one of the basal ganglia (Hawkins and Biebuyck, 1979). Also the metabolic meaning of the three KBs is different: while the main KB produced in the liver is AcAc, the primary circulating ketone is BHB. The third one, acetone, is produced by spontaneous decarboxylation of AcAc, and it is the cause of the classic “fruity breath.” Acetone does not have any metabolic functions, but it can be used as a clinical diagnostic marker. BHB acid is not, strictly speaking, a KB because the ketone moiety has been reduced to a hydroxyl group. Under normal conditions the production of free AcAc is negligible and this compound, transported via the blood stream, is easily metabolized by various tissues including skeletal muscles and the heart. In conditions of overproduction, AcAc accumulates above normal levels and a part is converted to the other two KBs. The presence of KBs in the blood and their elimination via urine causes ketonemia and ketonuria. Apart from being the fundamental energy supply for CNS, glucose is necessary for the replenishment of the quota of oxaloacetate, since this intermediate of the tricarboxylic acid cycle (TCA) is labile at body temperature and cannot be accumulated in the mitochondrial matrix. Hence it is necessary to refurnish the TCA with oxaloacetate via the anaplerotic cycle that derives it from glucose through ATP dependent carboxylation of pyruvic acid by pyruvate carboxylase (Jitrapakdee et al., 2006). This pathway is the only way to create oxaloacetate in mammals. Once produced by the liver, KBs are used by tissues as a source of energy (Fukao et al., 2004; Veech, 2004; McCue, 2010): initially BHB is converted back to AcAc that is subsequently transformed into Acetoacetyl-CoA that undergoes a reaction producing two molecules of Acetyl-CoA to be used in the Krebs cycle (Figure (Figure22).
Make things yourself. While it’s extremely convenient to buy most things pre-made or pre-cooked, it always adds to the price per pound on items. Try prepping veggies ahead of time instead of buying pre-cut ones. Try making your stew meat from a chuck roast. Or, simply try to make your mayo and salad dressings at home. The simplest of things can work to cut down on your overall grocery shopping.
The first group of 75 consumed a low-carbohydrate diet with less than 40 grams of carbohydrates per day. The second group of 73 consumed a low-fat diet with less than 30% of calories from fat and less than 7% of calories from saturated fat. Both groups regularly received nutritional counseling periodically throughout the study meeting with each participant meeting with a dietitian for a total of 10 sessions
Ketone esters (BHB-BD) increases activation of protein synthesis 31. Following exercise, complex signalling pathways within the muscle are activated to trigger the synthesis of muscle protein. A key regulator of muscle protein synthesis is a ‘kinase’ enzyme called mTOR. When athletes mixed ketone ester with a protein and carbohydrate recovery drink , muscle samples revealed that mTOR was much more strongly activated with ketone drinks compared with carbohydrate control drinks. This could help to promote gain of muscle mass occurring as a result of exercise.
I’m not sure if I really understand the idea of exogenous Ketones putting ones body into a state of KETOSIS, in that their own fat cells will be burned for fuel. Won’t the body just use the exogenous ketones and energy and continue to use glucose sources as well? How does taking ketones, make the body burn it’s own fat stores? I always thought that one would have to be in a state of energy deprivation for the body to actually begin to use the TG in the fat cells to generate ATP. Please let me know what I’m missing…
My dad was diabetic even though he was a competitive cyclist and ate "healthy". I grew up in a Weight Watchers household where everything was sugar-free and fat-free. Back then, if you had diabetes, you switched your white bread, rice, and pasta to whole wheat and you were fine. My dad died of pancreatic cancer when I was 19 years old. They say there’s no cause for this cancer, but I think the stress from his diabetes on his pancreas had a lot to do with it.
Note to all potential buyers: these are meant to support a ketogenic diet (no carb, high fat, moderate protein), a specific diet which takes time and dedication to engage in. These are not “silver bullets” that will cause rapid weight loss without diet and exercise, so don’t expect it. No such products exist, on Amazon or otherwise. On the other hand, if you’re following a ketogenic diet already and need a little boost every now and then, these are for you. Watch out, though, they have a lot of caffeine. Three pills have 200 mg, the equivalent of several cups of coffee. If you can tolerate it, go for it. Just don’t make a regular habit of it.
It also means that if you’re a very active athlete or exercise enthusiast and you’re following “trickle-down” advice from the sedentary or less active ketosis experts to eat less than 40g of carbs per day, you’re making a big mistake when it comes to your hormonal balance, and you need to up your carbohydrate intake to 100-200g of carbs per day. You’d be surprised at how easy it is (if you’re a very active person) to stay in ketosis on this level of carbohydrate intake. Go ahead. Do Ketonix breath testing to prove me wrong. You can eat boatloads of carbohydrates at night and be back in ketosis within just two to three hours. When you combine that with the cutting-edge tricks you’re about to learn, you’ll find that you can toss hormonal issues out the window, get into ketosis, have your cake, and eat it too. Literally.
In general, people on ketogenic diets tend to consume a lot of foods high in monounsaturated and saturated fats such as olive oil, butter (often butter from grass-fed cows is recommended), avocado, and cheeses. The high oleic types of safflower and sunflower oils (but not the regular forms of these oils) are also good choices, as they are high in monounsaturated fats and low in polyunsaturated fats.
Next, you need to ease yourself into this stuff. As I mentioned earlier, KETO//OS is blended with MCT’s, which can cause digestive distress if you’re not used to consuming them. This is due to the fact that your body has not yet adapted to the increased fats in your diet, and is less efficient at utilizing ketones as its fuel source. Once your body has adapted to MCT in the diet, the digestive distress will resolve. But I recommend you start slowly with just about a half a serving a day, and over two weeks, build up to a full serving twice a day.

Day 13: I have a love-hate relationship with this intermittent fasting thing. I think it's "working," and by that I mean I'm losing some weight. (Plus, improved body composition and definition can come with weight loss.) When I ask Dr. Axe if I should attribute my success to keto or IF, he says both. "I would say 80/20 it's more strongly in the favor of keto, but intermittent fasting does help as well," he says. The fat-burning capabilities of keto have more strength behind it when it comes to weight loss, specifically, he adds, but the intermittent fasting can be great for digestion and just feeling good.
-Cancer: Numerous studies have found that the risk for cancer increases with high blood sugar, which makes sense, since cancer cells feed primarily on glucose. This includes cancers of the endometrium, pancreas, and colon and colorectal tumors. Tim Ferriss recently hosted a fantastic article by Peter Attia about this very issue, and how ketosis may indeed be a potential cancer cure.
Sometimes it does get hard, sometimes you may want to cheat, and sometimes you do. I've done it. But remember your end goal - for me that is around 30 pounds to go, for you it may be to get stronger, or lose X amount of weight, etc., and get back on the horse if you fall down! A small Feb. 20, 2017, study looked at the impact of a six-week ketogenic diet on physical fitness and body composition in 42 healthy adults. The study, published in the journal Nutrition & Metabolism, found a mildly negative impact on physical performance in terms of endurance capacity, peak power and faster exhaustion. Overall, researchers concluded, “Our findings lead us to assume that a [ketogenic diet] does not impact physical fitness in a clinically relevant manner that would impair activities of daily living and aerobic training.” The “significant” weight loss of about 4.4 pounds, on average, did not affect muscle mass or function.
I've been researching the ketogenic diet for quite awhile so when I started this book, I was a little skeptical that there was anything else I could learn. I was wrong. Craig and Maria have done a wonderful job breaking down the science in a way that's easy to understand. Their knowledge and experience show you how to be successful on keto and how to break through plateaus so you can achieve your health goals. This isn't a "this is how keto works for me and my body so it should work for you too" book, it's a "this is how the human body works, how keto affects the human body, and how you can be successful with keto all based on science". There are also chapters dedicated to discussing the benefits of keto and how to heal your body - it's not all about weight loss. I knew keto was a good way to eat, but I had no idea how many syndromes, diseases, and even cancers that can be reversed through a clean keto diet.

The ketone bodies are possibly anticonvulsant; in animal models, acetoacetate and acetone protect against seizures. The ketogenic diet results in adaptive changes to brain energy metabolism that increase the energy reserves; ketone bodies are a more efficient fuel than glucose, and the number of mitochondria is increased. This may help the neurons to remain stable in the face of increased energy demand during a seizure, and may confer a neuroprotective effect.
Easy: use about half of the recommended serving of Glycofuse, and instead add one scoop of Catalyte electrolytes, one scoop of Kion Aminos, and one serving of ketones and/or MCT’s in the form of Brain Octane, KetoCaNa or KETO//OS (pick your poison, it’s up to you). While any of these forms of ketones and/or MCT’s works for daily focus and short workouts, I found that for long workouts they aren’t very gut friendly unless you really spread out the dosage (e.g. one serving every three hours), so you’d only really use that stuff in something like, say, an Ironman triathlon or multi-day adventure race.

It’s also a good option for those who show signs of a protein deficiency. Those signs include a loss of muscle or thinning hair, according to the subcommittee on the 10th edition of the federal recommended dietary allowances.
Want candy but you don’t want to stray from keto? A low-carb peanut butter cup has about the same calories, gram per gram, as a Reese’s. Want to slap all that bacon between something other than lettuce wraps? Some low-carb bread will run you $7.99 per loaf whereas bread that doesn’t taste like sadness with the same number of calories per slice is generally about half the cost. How about chocolate? A chocolate brand that boldly calls itself ‘The Good Chocolate’ is sweetened with a sugar alcohol commonly associated with some nasty gastrointestinal effects when consumed excessively. It’s also $8 for a 2.5-oz bar, and about as calorically dense as a Hershey’s bar. Low-carb flour? Not surprisingly, it costs more than normal old flour, and the more expensive one has more calories.
However, the researchers noted that it was still substantial and noted that “Evidence from this systematic review demonstrates that low-carbohydrate/high-protein diets are more effective at 6 months and are as effective, if not more, as low-fat diets in reducing weight and cardiovascular disease risk up to 1 year.”  They emphasized, however, that more research needs to be done to evaluate the long-term effects of the ketogenic diet on weight-loss.
“The Bulletproof Diet uses ketosis as a tool, but tweaks it for even better performance,” goes a blog post on the Bulletproof website. “It is a cyclical ketogenic diet, which means you eat keto for 5 to 6 days a week and then do a weekly protein fast, which lowers inflammation and kickstarts fat-burning. This is much better for your body and spurs weight loss even more.”
Enter the Internet. I tried a bunch of things… ordered this powder and that “organic concoction,” etc… nothing… I’m the cook in the family and my love for food has reared its beautiful head again. So in the spirit of finding a balance and battling my own obsessive issues, I eventually found a tea online that’s touted as a “detox” tea but I wouldn’t really call it that. I would describe it as doing the same thing for my obsessions that, say, chamomile tea does for relaxation. It literally “relaxes” my obsessions an mental “food shackles.” Yes, everything about it feels “salesy,” but I’ve tried literally a ton of these things and this one has had an extremely positive effect on my cravings to the point where I finally feel like I’m in control of my “food brain” again. Protein: Keep in mind that keto is high-fat, and not high-protein, so you don’t need to eat very much meat. Too much protein turns into glucose in the body, making it harder to stay in ketosis. Stick to fatty cuts of grass-fed, pasture-raised, or wild meat, and wild-caught fish. Red meats, offal/organ meats, pork, eggs (preferably pastured), fish, shellfish, and whey protein concentrate.
There are so many tricks, shortcuts, and gimmicks out there on achieving optimal ketosis – I’d suggest you don’t bother with any of that. Optimal ketosis can be accomplished through dietary nutrition alone (aka just eating food). You shouldn’t need a magic pill to do it. Just stay strict, remain vigilant, and be focused on recording what you eat (to make sure your carb and protein intake are correct).
Acetone is a molecule that results from the breakdown of acetoacetate. Acetone is commonly referred to as a ‘waste product’ as it is less readily used as energy compared to BHB (although some studies have shown that acetone can be oxidised as a fuel4. That said, some evidence suggests that it is responsible for the antiseizure effects of ketogenic diets so in may not be completely inert. At low levels acetone in the breath corresponds well to levels of ketones in the blood 12,13, however this is not the case as blood BHB levels increase 13 and if the increase is rapid, such as with exogenous ketone consumption11.
My goal with this blog is to help free you from the bondage that diets create. I want to help you learn to trust your body again and start loving the body you’re in. Because our worth is NOT determined by the size we are. Rather, let’s look at what type of human being we can be in this world. Compassionate, forgiving, loving, kind, generous, happy – these are the things that matter to me. These are things I want people to remember me by…not by how much I exercised or what I ate.
I’m a former low-carber, and while I did feel great while on keto, and it did help me to lose the biggest chunk of weight I have lost, I ended up finding that it was not a sustainable approach for me. If I ever slipped up, the bloating and weight gain that would happen overnight was so demoralizing (even if it was just water weight fluctuating up) that I would go into an emotional downward spiral complete with a crazy carb bender and gain everything back for real in a matter of weeks. I remember that at one point I was actually using test strips, and I got kicked out of ketosis by a meal of salmon and Brussel sprouts that had a sugary glaze on them, which I had chosen as a healthier option at a restaurant while my husband and my friends ate pizza. I was so angry that that had happened when I had been trying so hard to be good and I went right “back on the wagon” the next day, but I could not get back into ketosis, and I stopped losing weight. A consistent healthy pattern should not be so hard to maintain. I would tell people all day and all night that I didn’t feel deprived, and I didn’t think I was lying, but I think there was evidence in my behavior that I did indeed feel deprived.
I was diagnosed in April with type 2 diabetes. I have learned A LOT about carbs and how they affect a diabetic’s blood sugar. They are the enemy! I looked at the Keto diet and adapted parts of it to my dietary needs. I try to limit my carb intake to 20 grams a day, far less than what the American Diabetes Association recommends. I experimented eating a variety of carbs like oatmeal, pasta, fresh fruits, yogurt and all spike my blood glucose readings well above 30 points. Learning what I have I think it’s prudent for anyone to manage their carb intake closely, especially if they are NOT physically active. Exercise has a huge in pact in reducing blood glucose. My goal is to manage my diabetes without drugs instead with diet and exercise.
Maintain adequate protein intake. Too little protein and you lose muscle mass and starve the few parts of your body that can’t use ketones as an energy source, like portions of your red blood cells, kidneys and brain. Too much protein and you inhibit ketone production. Make sure you consume enough protein to support your vital functions, but not too much that protein becomes your alternate glycogen source.
