Some research suggests that ketogenic diets might help lower your risk of heart disease. Other studies show specific very-low-carb diets help people with metabolic syndrome, insulin resistance, and type 2 diabetes. Researchers are also studying the effects of these diets on acne, cancer, polycystic ovary syndrome (PCOS), and nervous system diseases like Alzheimer's, Parkinson's, and Lou Gehrig's disease.
Josh Axe, a doctor of natural medicine and clinical nutritionist, estimates that about 25% of people who try a ketogenic diet experience these symptoms, with fatigue being the most common. “That happens because your body runs out of sugar to burn for energy, and it has to start using fat,” he says. “That transition alone is enough to make your body feel tired for a few days.”

Let’s hold up a sec. Allow me to introduce you to the DIETFITS Randomized Clinical Trial, published in February 2018 in the Journal of the American Medical Association. DIETFITS, which stands for Dietary Intervention Examining the Factors Interacting with Treatment Success, succeeded in busting a slew of dietary theories. Microbiome is responsible for everything? Nope. Predisposition to success on one diet based on genetics? Nah. Low carb over low fat? Uh-uh.
It’s not for me, but it interesting to learn these things, and of course there seem to be good reasons for doing it for some people. I’m happy with the “eat less, exercise more” diet for now, but I might try out intermittent fasting since I’ve seen a few things suggesting it might help with allergies? I doubt that’s well supported, but I’ve liked what you’ve had to say about it, so since it’s not a thing I have to spend money on to try out, might as well, right?
I enjoyed reading this part of your blog. I’ve had a hard time starting keto. I need this kind of diet due to diabetes. My blood sugar was really high when it was last checked. I can do without most high starch foods like rice and pasta. Never been too fond of those two. My problem is potatoes and high refined sugar foods. (I love cake only for the real buttercream icing!) I’ve decided that I am going to make my carbs count. By that I mean natural sugar in fruit and veggies. When I eat my protein and “healthy” carbs and a bit higher fat, I do lose weight. I discovered that years ago. While I do want weight loss, keeping control of my blood sugar is far more important. In the past 10 years I’ve lost 80 pounds and have kept it all off. I want to lose 55 more, but will happily accept 25 more off. My want for weight loss is for my health and cheaper (and prettier!) clothing. Your struggle has shown me that I may have to do what I’ve been trying- finding a “compromise” diet that works for me. Thanks for sharing your struggle.

The gut-brain link is important not only for the hormones produced by the gut, but also for the long-term body weight regulation. Studies in mice indicate that the gut microbiome influences both sides of the energy balance by contributing to nutrient absorption and regulating host genes that affect adiposity [however there are conflicting reports (Parks et al., 2013; Schele et al., 2013)]. However, it remains uncertain just how important gut microbiota are for nutrient absorption in humans. A cohort study has demonstrated that the nutrient load is a key variable that can influence the gut/fecal bacterial content over short time frames. Furthermore, the observed associations between gut microbes and nutrient absorption indicates a possible role of the human gut microbiota in the regulation of the nutrient intake and utilization (Jumpertz et al., 2011).
When the kidneys filter blood, metabolic substrates such as glucose and ketones are re-absorbed to prevent energy wastage. If blood levels of a metabolite exceed the capacity of the kidney to reabsorb them, then a ‘spillover’ effect occurs and the metabolite (i.e. glucose or ketones) appear in the urine. However, urine is not a very reliable measure. Firstly, whilst following a ketogenic diet, adaptation occurs over time that means more ketones are reabsorbed in comparison to the early phase of the diet9. Furthermore, at higher levels of ketones, the appearance in the urine does not correlate to levels in the blood10. Similarly, after consumption of exogenous ketones, urine ketone levels were not in proportion to the levels in the blood11 this may be because of the rapid onset of ketosis in comparison to when ketosis is achieved with fasting or diet. Therefore urine test strips are useful as a guide but have several disadvantages to their use to accurately quantify levels of ketosis. 
But I did NOT love being constipated despite plenty of fibrous leafy greens and increased water consumption. As a nurse I know how important regular bowel function is and as I just couldn’t get around this one I have decided to loosen up a bit and follow the CSIRO low carb guidelines which allow a bit more carb, less fat and is way more bowel friendly. Hoping that it doesn’t affect those ghrelin levels too much.
Sorry, I guess I read your comment wrong. You are wondering what to do about sugar levels, weight, and enjoy life. Personally, I have found that eating mindfully and listening to YOUR body is your best resource. No diet can tell you what to do, only your body can do that. Do you feel good eating more carbs? Do you feel better eating less carbs? Do you feel better fasting or not? Your body will tell you what to do. Also, I want to ask, what is the reason for wanting to lose weight? Is it purely health? Have you had tests that tell you your health is bad? Sometimes we get stuck on a specific number for what we “want” to weigh, which actually has NOTHING to do with out health. I recommend the book, Health at Every Size if you want to learn more about that. All of us have a weight set point, and for some, that is higher..which may be hard to accept. If I were you, I would take the focus OFF of weight loss completely and focus on eating in a way that nourishes you and makes you feel good and happy. Life is too short to focus on what is “right” and “wrong.”
Blood tests often report the level of total cholesterol (HDL + LDL) as well as the levels of each type independently. It is possible that the relative abundance (ratio) of HDL: LDL is more important to predict the occurrence of cardiovascular disease that the total cholesterol level109. Whilst the ketogenic diet can cause an increase in total cholesterol, the ratio of healthy HDL : less healthy LDL generally increases (i.e more HDL)110 whilst following a ketogenic diet. In contrast, whilst total cholesterol tends to be lower whilst following a low fat diet, the ratio of HDL:LDL tends to be lower (i.e more LDL)21. 
And here’s the reality of the situation. Not all carbs are created equal. There’s a lot of cleverly labeled and modified maltodextrin on the market claiming to be some technical polysaccharide. Or, there’s plain old cornstarch relabeled as super-duper muscle engorging waxy maize. Most of these aren’t proven to do anything except deliver false hopes, high blood sugar, and gut issues – as opposed to having multiple clinical studies in real athletes to support what it is they’re claiming.
IBS and even though I go for periods of time feeling well, it does flare up and drives me crazy. My husband, 66 years old has been doing the Keto diet for 5 months and have lost 25 lbs. (he has12 more to go to be on a healthier weight). I am his main support with Keto as I cook in a very creative and I vary his weekly menu a lot to make it Keto friendly and healthy. He is doing about 35-38 grams of carbs per day that allows more vegetables and berries. He has become extremely sensitive to sodium and gets bloated, therefore now also doing low sodium diet.
I am so glad that I never went full board on the keto diet. I work from home. Because of this, I have the option to eat my bigger meal at lunch time rather than dinner/supper. What I noticed for me personally is that when I go to bed on a fully carbed meal, I am less likely to want to eat breakfast in the morning. So, I have moved my carbs to lunchtime and then a dinner that is lighter and less carbed out. I also go to the gym in the evening and just don’t care for a heavy meal sitting on my stomach because I feel like I have to work out just to burn that meal off.
Hello, I am doing IF 16:8 & my eating window is in the evening. I’m confused as to what to eat during those 8 hours. I’ve been eating 1 meal & a snack. Is this enough? I’ve been doing Keto dieting for 6 months. Initially lost 5 #. (Realize it was water #) Now I weigh more than I ever have, as of today!! I had trouble at the end figuring out if you were now eating high fat (60-80% of calories), or if you went back to something closer to 50% fat calories and higher carbs and are using exogenous ketones to get int ketosis when needed. Are you doing any carbs at night or a refeed, or just having higher carbs on most days? Thanks
How It Works You’ll follow the keto diet as usual until 30 to 45 minutes before exercise — then it’s time to eat about 25 g of carbs, says Daniela Torchia, PhD, a registered dietitian based in Loma Linda, California. The idea is that you’ll have just enough carbs to fuel your workout and still be able to return to ketosis easily after you cool down. For the relatively new keto diet, the experts were especially concerned about extremely high fat content -- about 70% of daily calorie intake -- as well as unusually low carbohydrate levels: only 15 to 20 net carbs a day. The 2015-20 dietary guidelines for Americans suggest that 45% to 65% of daily calories come from carbs but less than 10% from saturated fat.
I loved this post, thank you. I tried KETO for 1 day only and I felt terrible. It just wasn’t worth it to me to feel like shit even for one day and I knew I had to study a lot at the time, so I decided that keto is just not for me. But at the same time I felt bad for feeling this way, like something was wrong with me, because I couldn’t even follow a diet, that many people say it’s very easy to follow. And now I just accepted the fact that I love carbs too much to give them up (especially fruit).

The trick with the rice is to find a substitute, and we’ve already looked at cauliflower rice. Avoid over grating your cauliflower. You don’t want it so fine that it is a fine powder for this dish. It still needs to have a rice texture. You will also need to add some cream cheese to the cauliflower to work for this rice substitute. Otherwise you’ll just get the cauliflower everywhere!

A few days ago I started reading about Keto and what carbs really do to you. Figured It could be one more diet to try. Simply take every single carb out of my diet and just eat the rest? Even someone as stubborn as me could do that. So I’m Basically eating eggs meat cheese and water. A diet Dr Pepper with dinner. On oct. 7th 3am I made the choice to start I weighed myself at 524. It’s now Oct 10th at 1:30pm and I’m 505. (I’m also walking around for a total of 2 hours per day) I’m finding it hard to believe. Ketosis is a metabolic state where most of the body's energy supply comes from ketone bodies in the blood, in contrast to a state of glycolysis where blood glucose provides most of the energy. Ketosis is characterized by serum blood concentrations of ketone bodies over 0.5 millimolar with low and stable levels of insulin and blood glucose. However, with ketone supplementation (as you’ll learn about later in this article) ketosis can actually be induced even when there are high levels of blood glucose.
2) I have a hard time eating real food soon after rides/workouts. I had used Hammer Recoverite (1scoop instead of suggest serving of 2 and add 1scoop whey isolate protein) in the past because I feel a significant difference the day after with less muscle fatigue. Then I read a previous posts on your opinion of post-workout supplementation ( https://bengreenfieldfitness.com/2013/07/what-to-… ) and I realized the primary ingredient in Recoverite is maltodextrin. Since the 2013 article, has anything changed in your research that you might suggest I add PWO to aid in the muscle fatigue/recovery? (In other words, Is there anything more healthy I can take to replace the Recoverite or should the aminos/electrolytes/carbs/MCT’s from the recipe in this article be sufficient?) Many low-carbohydrate diets tout the benefits of being in a state of ketosis, which they claim results in weight loss. The idea is that by not eating enough carbohydrates to use as energy, your body will instead break down fat for energy. As a result, you will lose fat and weight. The idea of ketosis for weight loss, however, does not work as well in reality and comes with many health risks. Talk to your doctor before going on a diet that promotes ketosis.
Specific fiber goals for every day will depend on your overall intake, current weight, and weight-loss intentions. Thankfully, some high-fat, low-carb foods are also loaded with fiber. These include nuts and seeds, avocado, and squash. “I see so many clients go for high protein, high saturated fat, and no carb,” says Sunny Brigham, MS, CNS, a board-certified nutrition specialist with a private clinic in North Texas. “They become constipated because they aren’t getting enough fiber.” And that’s just one of the 11 hidden dangers of the keto diet.
If you’re serious about maximizing the benefits of ketosis, then forego coconut oil, MCT liquid oil, olive oil, etc. and instead use Brain Octane as your oil of choice for recipes like bulletproof coffee, or in teas, salad dressings, or as a sushi or entrée flavor enhancer. In recent times there has been an exponential increase in the rates of obesity and diabetes. Popular opinion has blamed (in turn) overconsumption of fat, overconsumption of carbs and sugar and overconsumption of calories. Thanks for sharing this information and your experience. I started Keto because I was having severe digestive issues where nearly everything seemed to make my tummy hurt. I saw that alot of the recipes and choices were gluten free since low carb, so I figured I’d try it and hopef5find some foods I COULD eat! I did strict for the first few weeks til I could see how it worked out and did find great relief in that there was nearly ZERO pain and bloating. Now I don’t do KETO, but I continue to substitute alot in the same manner… and I cook breads made with almond and coconut flour especially. I keep low sugar intake, but not nearly zero, and I try to keep within healthier carbs so no more pasta for instance, but maybe a little rice or sweet potatoes etc. I did start having hormone symptoms when doing strict keto and getting very tired also. I lost about 14 lbs right off then gained back about 5. I am starting to feel like I’m finding a good balance that works for me now, and I am thankful to have opened up some new nutrition ideas that my body seems to like better! I will say, I joined one fb group for KETOers, but found it to be very stressful as lots of snotty comments and attitudes… maybe isn’t that way in the entire KETO community, but was very off-putting! The biggest draw for me is how many of those who’ve tried it say they don’t get hungry. The possibility of that blows my mind, as someone who’s used to any sort of calorie restriction meaning hours of feeling hungry every day. It’s tantalizing enough that I at least want to try. And what have I got to lose, right? I’m already morbidly obese; it’s hard to imagine screwing this up so bad it makes that worse.
In many developing countries, the ketogenic diet is expensive because dairy fats and meat are more expensive than grain, fruit and vegetables. The modified Atkins diet has been proposed as a lower-cost alternative for those countries; the slightly more expensive food bill can be offset by a reduction in pharmaceutical costs if the diet is successful. The modified Atkins diet is less complex to explain and prepare and requires less support from a dietitian.[54]
In contrast, multivitamins aren’t a good solution as they are synthetic and lack a lot of nutrients like polyphenols, antioxidants and fiber that green powders and whole food sources provide. And the lack of fats and enzymes make the nutrients they do contain very difficult to process properly. Just because you’re putting something in your body doesn’t mean your body can use it.

I am curious if someone takes any or too much MCT oil (5-10 tablespoons/day) or Exogenous Ketones will the liver slow/stall or shut off endogenous ketone production as you are providing the ketones exogenously and thereby you may NEED to supplement ketones to maintain ketone levels or you may suffer a short term ketone deficit while the liver adjusts to making them on its own again. A parallel I am thinking of is exogenous testosterone supplementation and the hypothalamus/endocrine system slowing/stopping endogenous testosterone production (seen in bodybuilders).
I will admit to appeal to authority here. This was said by the professor of the course I mentioned in my previous post, but it was also confirmed by many of my classmates, whom, at this point(for reasons, which are too tedious and long winded to extrapolate on atm), I consider smart enough to know their business, that I choose to believe them. The ketogenic diet: The ketogenic diet could affect cancer by a similar mechanism to calorie restriction and fasting. Of the 29 animal studies that have examined ketogenic diet and cancer, 72% of them have demonstrated an anti tumor effect. There have been a small number of case studies of the safety and efficacy of the ketogenic diet in humans. The results have been largely positive, for example following a ketogenic diet helped to stabilise tumour growth and alter tumor metabolism96 ,97 ,98. There is an absence of fully randomised clinical trials in humans, but 24 case studies have been published; one recent appraisal of the case study data said that “ a probabilistic argument shows that the available data strengthen the belief in the anti-tumor effect hypothesis (of the ketogenic diet) at least for some individuals"99. 
Advocates of the keto diet will say that while their total cholesterol will increase, it’s only the good kind (HDL) that increases, not the bad kind (LDL.) However, a study using the keto diet as a form of treatment for epileptic seizures in children found that after 6 months of administering the diet, triglyceride levels increased, total cholesterol levels increased, and HDL and LDL increased. So if heart disease runs in your family, you may want to do more research before jumping on the Keto bandwagon.

Ketone esters: These are the raw ketones (in this case, beta-hydroxybutryate) that are not bound to any other compound. These exogenous ketones can be utilized quicker and potentially have a better effect at raising blood ketone levels as your body doesn’t have to cleave the BHB from any other compound. The downside? They typically have a foul taste and can cause an upset stomach.
This benefit surprised me when I first discovered it, but eating fewer carbohydrates during a workout can actually help you recover from workouts faster. The repair and recovery of skeletal muscle tissue is dependent on the “transcription” of certain components of your RNA. And a bout of endurance exercise combined with low muscle-carbohydrate stores can result in greater activation of this transcription. In other words, by training in a low-carbohydrate state, you train your body to recover faster.

The good thing is that there are supplements that can help reduce the adverse effects of a specific diet on the human body. You also get incredible gains in metabolic efficiency when you use fat as a primary source of fuel – especially when doing high-intensity interval training – with this one-two combo causing potent 3–5 percent decreases in the oxygen cost of exercise, which is extremely significant. Translated into real- world numbers, this increased fat utilization from carbohydrate restriction and high-intensity interval training would allow you to pedal a bicycle at a threshold of 315 watts, whereas a high-carbohydrate, aerobic-only program (the way most people train) would allow for only 300 watts. Talk to any cyclist and you’ll find out that an 15 extra watts of power is huge in a sport like cycling, and something most cyclists train years and years to achieve.
Like the ketogenic diet, the subjects aimed to eat 20 grams of carbohydrates per day or less for a 2-3 month induction phase; then, they were asked to eat 50 grams of carbohydrates daily for the following 9-10 months. All participants were instructed to maintain a calorie deficit and utilize professional support to adjust to their diet and make sure that they stayed healthy. Additionally, the research team emphasized general health-promoting behaviors such as regular exercise and using nutritional supplements.

This is in part because patients need to be in deep ketosis to see an impact on epilepsy, likely deeper than the average dieter, but without a nutritionist guiding you it’s still hard to get down into ketosis. It’s not exactly clear why ketosis seems to improve epilepsy, but it seems to have something to do with the brain’s use of ketone bodies in place of glucose, which only happens when you’re nearly in starvation mode. (It's important to note here that "starvation mode" is not in reference to how hungry you feel.)

The child must eat the whole meal and cannot have extra portions; any snacks must be incorporated into the meal plan. A small amount of MCT oil may be used to help with constipation or to increase ketosis.[36]

Many neurological conditions share a common feature of impaired brain energy metabolism. It isn’t always clear if this impairment is the cause or the effect of the disease, but nonetheless, interventions that even partially restore or improve brain energy metabolism could help to prevent, slow or even reverse some conditions of the brain. Because ketones can: 1) get into the brain; 2) undergo metabolism by a distinct pathway that bypasses glucose metabolism, providing ketones by either following a ketogenic diet or by taking exogenous ketones could impact the natural course of some neurological conditions.

Ketone esters (BHB-BD) lowers blood lactic acid 30. Lactic acid build up occurs during exercise as a result of burning carbohydrate at a high rate without enough oxygen. Blood lactic acid levels during exercise were 30% lower after ketone ester drinks compared to carbohydrate drinks. This is because high blood levels of BHB from the ketone ester drink slow down carbohydrate use and increase oxygen efficiency, which could decrease blood lactic acid levels.

The Ketogenic diet is not about cutting all carbs, just cutting way back on carbs. We have gone and had regular check ups at the doctor with no negative results. I for one have always had an issue with poor digestion but since switching to the keto diet that has drastically changed. The hardest part for me was to cut pasta but i found great joy and fun in finding alternatives to pasta so i can still enjoy the same dish minus the carbs. There are so many different diets out there and so many different types of people so you have to find the right one for you, the keto diet is not for everyone but for us, so far it seems beyond perfect. To understand exogenous ketones, you should know that there are three types of ketones: beta-hydroxybutyrate (BHB), acetoacetate (ACA) and acetone, and all three are the normal by-products of fat breakdown by your body. In much the same way as glucose, ketones can be used by your tissues, especially your brain, diaphragm and heart and are actually a far more efficient fuel source than glucose.


There is a transition period in ketosis while the body is adapting to using fats and ketones instead of glucose as its main fuel. There can be negative symptoms during this period (fatigue, weakness, light-headedness, headaches, mild irritability), but they usually can be eased fairly easily. Most of these symptoms are over by the first week of a ketogenic diet, though some may extend to two weeks.

Also, when I read what you’re eating, that doesn’t seem like nearly enough!! I’m not one to promote counting calories, however, I do feel that it can be helpful to see if you are eating enough. That doesn’t sound like it’s even close to 2,500-3,000 calories. Yes, you read that right…2,500 – 3,000 calories!!! When we don’t eat enough, our metabolism slows and no matter what we do, we won’t lose weight.
Day 5: As fate would have it, 3 p.m. rolls around and we get a message that there are cookies in the conference room. I have been snacking on keto-approved foods like Granny Smith apples (the tart green apple has way less sugar than, say, a red Gala), and full-fat cottage cheese with blueberries (where have you been all my life, snack?) with no real trouble with cravings. But just knowing there are cookies that I can't eat makes me feel a little cheated.  (Though These Low-Carb Keto Desserts Help With That.)

Yes good fats can in some people raise LDL ( which is NOT bad cholesterol unless it’s oxidized) I’ve never heard of the Keto diet elevating triglycerides IF it’s done right. Never! If it’s done right, triglycerides go down, usually way down, LDL MIGHT rise in some people but HDL goes up and if you get the correct cholesterol test which breaks down LDL into particle size and number, you’ll find that even if it’s elevated, it’s made up of mostly large fluffy LDL,s instead of the harmful small dense LDLs which damage vessel walls. IF Keto is done right the HDL to triglyceride ratio ( most important numbers on a regular lipid panel) is very low- and that’s excellent.
It is known that different dietary components exert some effects on gut microbiome composition, mainly in relation to obesity and inflammatory states. In general, a Mediterranean diet has a positive effect while a high-protein diet seems to have detrimental effects due to putrefaction phenomena (Lopez-Legarrea et al., 2014; Flint et al., 2015). Few data are available at this time about the effects of KD on gut microbiota. For example, a study by Crawford et al. (2009) investigated the regulation of myocardial ketone body metabolism by the gut microbiota and demonstrated that, during fasting, the presence of gut microbiota improved the supply of ketone bodies to the heart where KBs were oxidized. In the absence of a microbiota, low levels of KB was associated with a related increase in glucose utilization, but heart weight was still significantly reduced. The myocardial-mass reduction was completely reversed in germ-free mice feeded with a ketogenic diet. Regarding food control we can hypothesize that the particular metabolic state of ketosis could provide some benefit to weight and food control via synergic actions between butyrate production by gut bacteria and circulating high blood ketones (Sanz et al., 2015).
